Interview Sheet and Permission Slips 01-27-2012

ATTENTION

Email me at jzimmerman@k12tn.net so that I can send you an electronic version of the interview sheet.

I need these sent to my email so that I can email them to the state competition people.  If these are not turned in by the 30th (along with the permission slips) or our team may not be able to compete.  Which means that one person can cause an entire team to have a reduction in score or worse.

Also make sure that the permission slips have been turned into Ms. Mathis before lunch 3rd Block TODAY.

Extra-Curricular Activities

NAME

This information will be used as background material by the Academic Decathlon interview committee. PLEASE PRINT OR TYPE ALL INFORMATION. Be complete. Have your counselor or school administrator sign the form.

Include all outside activities you have participated in during your high school years. The number of extra-curricular activities will not affect your final score. If there is not enough space, attach a second sheet. Please fill out three copies of this form.
